Dr. Stanley Jack has been in private practice since 2007. After receiving a B.S. in Biology and a B.A. in Psychology at the University of Colorado at Denver, Dr. Jack attended medical school at Des Moines University College of Osteopathic Medicine and Surgery, during which he did a year of clerkships in the Cleveland Clinic Health System. He then went on to complete an internship in Internal Medicine and his residency in Family Medicine, both at Exempla Saint Joseph Hospital in Denver, Colorado.
Prior to private practice, Dr. Jack provided care in the ambulatory setting as a Family Physician and an Urgent Care Physician. Other areas of medical interest include Sports Medicine, Osteopathic Manipulative Medicine (OMM), and Wilderness Medicine. Dr. Jack is an Associate Clinical Professor for the Rocky Vista University College of Osteopathic Medicine, and is a Basic Life Support and Advanced Trauma Life Support Instructor.
Patricia Henman is a Colorado native and a Board Certified Physician Assistant who began her journey into medicine as a Certified Nurse Assistant and then a Medical Assistant during her high school and undergraduate years. After receiving her Bachelor of Arts in Biology and Chemistry from Carroll College in Helana, Montana, she then spent several years in medical research at National Jewish before returning to school. In 2007, she completed a Masters of Medicine degree through St. Francis University and the Physician Assistant program at Red Rocks in Golden, Colorado. She has been practicing in Family Medicine ever since.
Patricia has a special interest in Women’s Health, particularly early detection and prevention of common female cancers. She is also experienced with bioidentical hormone replacement for women.
Natalie Ibrahim completed a Master's degree in Physician Assistant Studies at Christian Brothers University in 2016. She received her B.S. in Biology from the University of Memphis in her hometown of Memphis, Tennessee. During college she worked as a Hospital Pharmacy Technician, and after her graduation she did research for a few years at St. Jude Children’s Research Hospital. In 2017 she and her husband moved to Colorado where she began her medical career at Elkridge Family Physicians in Evergreen. She joined Boulder Valley Family Practice in 2021.
Natalie has a special interest in preventative care and enjoys empowering patients to improve their overall quality of life through education and solutions that fit their unique lifestyles. In her free time, she loves spending time with her husband and twin boys enjoying the outdoors.
Darrell Creekmore attended California State University Stanislaus as an undergraduate where he earned a Bachelor of Science in Biology and Chemistry, graduating with honors. He became a Physician Assistant in 2017 after earning a master’s degree at Samuel Merritt University in Oakland, California. Prior to Family Medicine, Darrel worked in Emergency Medicine and Urgent Care.
Darrell practices individualized care and believes the pursuit for health begins with treating the human body as a whole. He holds special interest in sports medicine, ergonomics and postural therapy, nutrition, sleep, the mind-body connection, and the oral-systemic link. When he is not seeing patients, he enjoys hiking, yoga, and cooking for his wife.
